Case Manager (Registered Nurse)

JOB DESCRIPTION

Position Title:   Case Manager (Registered Nurse)
Department: Home Health/Hospice – Clinical Operations
Reports to: Clinical Supervisor &/or Hospice Director/PCC

Position Summary:

Registered Nurse case management designed to improve individual and/or family quality of life to a specified client group and their families. Coordinates comprehensive health care and community-based services, home visits, and assessments. Develops, implements, coordinates, and evaluates health care plans in an outpatient environment; collaborates with other health care providers and community service agencies to coordinate care.

EDUCATION:   Must have current RN License
EXPERIENCE: Minimum of 1 year home health care or related experience
REQUIRED SKILLS: Detail oriented, organized, ability to multi-task
PREFERRED SKILLS: Self-starter, ability to work alone under little supervision
