Position Summary:
Registered
Nurse case management designed to improve individual and/or family
quality of life to a specified client group and their families.
Coordinates comprehensive health care and community-based services,
home visits, and assessments. Develops, implements, coordinates, and
evaluates health care plans in an outpatient environment; collaborates
with other health care providers and community service agencies to
coordinate care.
Essential Job Functions:
- Coordinates, monitors, and evaluates provider services and
care, to include home visits, physician/hospital visits, and therapy
visits, for a specified client population.
- Documents all client encounters and contracts made on
behalf of clients/families; writes social and/or nursing summaries.
- Coordinates services and attends regular staff and
management meetings.
- Evaluates and documents the progress of individual
therapeutic programs, and makes modifications, as required.
- Performs a range of associated administrative functions, as
appropriate, such as preparing administrative and programmatic reports
and correspondence and submitting billing documentation, as required.
- Evaluates reports and identify areas needing improvement;
make necessary improvements.
- Provides direction and coordination of all patient care,
including other visiting nurses and MDT members.
- Ensures all OASIS documentation, and other appropriate
assessments, POC, subsequent orders communicated regarding patient care
and change in patient condition are communicated, coordinated,
documented and executed.
- Participates in all patient related care meetings and
decision making processes.
- Utilizes decision-making and critical-thinking skills in
the review and determination of coverage for medically necessary health
care services.
- Acts as liaison between the beneficiary and insurance provider to utilize appropriate and cost effective medical resources within the direct care and purchased care system.

| REQUIRED SKILLS: | Detail oriented, organized, ability to multi-task | |
| PREFERRED SKILLS: | Self-starter, ability to work alone under little supervision |
